I don't think we have done one of these for a while so I thought I would dig out the old form and put it up for people to fill out.

Pick your five favourite games, or less if you don't have five, they will be weighted by where you place them. Also a few other questions that you are free to ignore if you don't feel like filling them in. Deadline is Friday of this coming week and then I'll put the results up over the weekend.
