This strange apparition is acting outside of normal combat initiative procedure, so anyone may do anything at any time, providing they're not currently required to roll for an initiative test (which Malaiz currently is).

Yes, it will.
Very well; Fechr falls through the pit-trap and lands at the bottom of a six-foot deep shaft.
Thankfully, due to his sturdy, war-hardened frame (and an Omen) he is unhurt. But he's still trapped at the bottom of a six-foot shaft. You can attempt to climb out by testing Agility (d20).
